Personal Care

The Personal Care program aims to assist eligible Clients with showering, grooming and/or therapy activities, minimal household tasks such as assisting with meals and/or meal preparation. This service is available to people in stable health who can be safely assisted without the need for significant medical/nursing services. Where case management and support is provided by Community Nurses, Disability Services or other agencies Coordinators may agree to provide personal care to people of less stable health.

Respite Care
The Respite Care Program aims to assist the Primary Carer of a person with a disability to have time out. Respite is offered in a Client's own home or within the local community. Respite Care is usually provided on a regular basis but may also be provided on an occasional basis, depending on sufficient funded hours being available.

Hosting Program
The Hosting Program enables regular or occasional support being provided in the Host's home thereby enabling people with disabilites to participate in other families activities and primary carers to have much needed time to do other things. Rural and Remote Program

The Rural and Remote Program is founded on three principles:

1.That people with a disability have the right to live where they choose without being unfairly disadvantaged through lack of appropriate services.

2.That all people with disabilities have the right to make the decisions that affect their lives.

3.That all people with disabilities have the right to support to enable them to maintain their independence at home and within their personal and social networks in their chosen community.

Family Based Care offers a program specifically designed to assist people with permanent or long term disabilities who live in isolated or rural communities and who currently have few or no appropriate community services available to them. The Rural and Remote program offers respite hours, which can be utilized in a flexible manner by negotiation with the client and their family.

People with disabilities have goals, desires, ambitions, wishes, hopes and dreams as do the rest of the community. As with the wider community, these a spirations span from something as simple as being able to go to the movies through to attaining a qualification that will allow participation in the workplace. Some have wider horizons due to their own personal outlook or the supports that are in place. Others, however, have barriers and limitations to such a degree that they have difficulty thinking about tomorrow or next week, much less the future.

Living in a society that is often seen as inaccessible, exclusive and not accepting of people with a disability can have the effect of lowering aspirations and expectations and stifling dreams. When people are further isolated through distance, the barriers can become insurmountable.

Family Based Care seeks to address this imbalance for our clients by providing the avenue to encourage active participation in the community in all areas of interest.

Mental Health Program

The Mental Health Program has been a very successful initiative and has been in place since June 1994 as part of a collaborative agreement with State Mental Health Services. The Program aims to provide planned support to ind ividuals with a mental illness and their family. It allows people to be supported in their own home or within the community. ReferralsĀ are to go through the Maximising Recovery Panel before they can be accepted into the Mental Health Program through Family Based Care. Dementia Carer Respite Program
The Dementia Carer Respite Program is a specialist program that complements available generic services and endeavours to provide in-home respite for the primary carers of people with dementia. The program also provides personal care, support, education and skill development. Our aim is to support and work with the carer to help them develop strategies that may help improve the quality of life in the home for all concerned.

Spring Cleaning Program

The Spring Cleaning Program aims to provide a once-a-year spring clean to HACC eligible clients and is provided to assist clients to maintain a safe home environment. Assistance is dependent on funding and there is a limit to the amount of assistance that is available. This program aims to help with tasks that clients can no longer perform for themselves and are not usually covered by existing Home Help services. Each referral is assessed and assistance is provided according to the needs of the client. Costs are in accordance with National HACC fees policy and this will be discussed with you at the time of referral.

Dementia Carer Support Service
The Dementia Carer Support Service provides professional counselling and support for those individuals who are caring for a person with dementia at home. The service is provided in recognition of the enormous demands placed on those people, including relatives and friends, who are endeavouring to care for people with dementia. Through information, support and counselling the service aims to enhance the capacity of carers to continue to provide care at home. Carers can self refer or be referred by family, a General Practitioner or other health professional with the Carers permission. Regular support group meetings are held in both Devonport, Burnie and Smithton where carers are able to share their experiences and gain peer support. The service also provides education programs for the broader community on dementia and the needs of carers.

Home Maintenance Advisory Service
The Home Maintenance Advisory Service is a unique program which aims to assist HACC eligible clients to maintain, remodel or alter their residences to suit their changing lifestyles or disabilities with an affordable solution. The service provides support by ensuring that any appropriate maintenance is carried out economically by reputable trades people. The HMAS officer can visit the property and establish the works required in conjunction with the owner, and also prepare specifications and cost estimates for that work. Information can also be provided on quotations and selecting a suitable trade organisation / tradesperson. A visit after the work has been completed to determine that it adheres to the specified standard is available. There is no charge for advice provided through this program.

Veterans Home Care Program
The Veterans Home Care Program offers packages of care to eligible Veterans and war widows who are card holders of the Department of Veterans Affairs. The package may include Domestic Assistance, Personal Care, In Home Respite and Home and Garden Maintenance. Veterans eligible for this service are assessed for access to the program by Veterans Home Care Assessors by phoning 1300 550 450
